Job summary

MyCHN Home Health Services is urgently hiring a skilled Physical Therapist Assistant (PTA) to support patients in their homes under the supervision of a licensed Physical Therapist. You will help patients achieve rehabilitation goals, improve mobility, and enhance quality of life through structured exercises and education.

The ideal candidate is a graduate of an accredited PTA program with a current Texas license and a commitment to compassionate, patient-centered care.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current Texas State License as a Physical Therapist Assistant.
• CPR certification preferred.
• Minimum one year of experience in home health, acute care, rehabilitation, or related setting preferred.

Responsibilities

• Provide physical therapy treatments and interventions as outlined in the Plan of Care by the supervising PT.
• Observe, monitor, and document patient responses to treatment and communicate findings to the PT.
• Assist patients in improving mobility, strength, balance, endurance, coordination, and functional independence.
• Instruct patients and caregivers on prescribed therapeutic exercises, home exercise programs, and safety techniques.
• Recognize changes in patient condition and promptly report concerns to the supervising PT and care team.
• Maintain accurate and timely clinical documentation per agency policies and regulatory requirements.
• Assist with evaluating and recommending durable medical equipment and adaptive devices as directed by the PT.
• Apply infection control principles to prevent contamination and ensure patient safety.
• Participate in mandatory continuing education and in-service programs.
