Qualifications

• Bachelor or Master of Science in Occupational Therapy.
• Licensed Occupational Therapist.
• Basic Life Support (BLS) certification.
• OT Certification.

Responsabilités

• Performs Occupational Therapy evaluation, reassessment and consultation upon referral.
• Provides plan of care and discharge planning; secures physician certificate for the plan of care.
• Provides OT services per AOTA guidelines and regulatory requirements.
• Participates in discharge planning when requested.
• Communicates with patient’s physician, nurse and case manager regarding evaluation, progress and discharge plan.
• Ensures patient safety, patient rights and HIPAA compliance.
