Job summary

Humana Inc. through CenterWell Home Health is seeking a Home Health Speech Language Pathologist to deliver in-home SLP services.

You will evaluate patients, develop and review treatment plans, and provide therapy for speech, voice, language, hearing and swallowing disorders in home or facility settings. You will also document evaluations, coordinate care, and train patients and families while maintaining confidentiality and professional standards.

Qualifications

• Must hold ASHA CCC or equivalent in Speech-Language Pathology or Audiology.
• Minimum six months experience as a speech therapist/speech language pathologist.
• Current and unrestricted license to practice.
• Current CPR certification.
• A valid driver’s license, auto insurance and reliable transportation.

Responsibilities

• Evaluate, direct and provide speech/language pathology services to patients in their home or facility.
• Participate in development and periodic review of Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing; administer and interpret tests.
• Observe, record and report changes in patient condition and response to treatment.
• Provide instruction in alternative communication systems when appropriate.
• Maintain and submit documentation and reports as required by policy.
• Participate in care coordination and discharge planning.
• Maintain confidentiality and ethical handling of patient information.
