CenterWell Home Health is seeking a dedicated Speech Language Pathologist to deliver in-home evaluations and therapy, contributing to care plans and improving patient communication, swallowing and cognitive skills.

You will work under the Clinical Manager, documenting progress, coordinating with care teams, and educating patients and families to promote independence and quality of life.

Formación

• Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA.
• Minimum six months experience as a speech therapist / speech language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• A valid driver's license, auto insurance, and reliable transportation are required.

Responsabilidades

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to prevent and minimize effects.
• Administer and interpret diagnostic tests and applications of therapeutic treatments including audiologic screening.
• Observe, record and report changes in the patient's condition to supervisor or physician.
• Provide instruction and training to patients in use of alternative communication systems when appropriate.
• Provide counseling and instruction to patients, families and healthcare staff.
• Maintain and submit documentation; prepare reports and progress notes as required by policy.
• Participate in care coordination activities and discharge planning.
• Maintain confidentiality and handle information appropriately.
