Job summary

Enhabit Home Health & Hospice is seeking a dedicated licensed professional to provide skilled care in patients' homes. Each day you will educate patients and families, collaborate with the health care team, and carry out services per physician orders and the established plan of care (POC).

Ideal candidates are graduates of an approved clinical education program, licensed in the state of practice, with at least one year of licensed professional experience and basic tech skills including mobile

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed professional.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Administer skilled care to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
