Qualifications

• Maryland Physical Therapy license required.
• Current CPR certification.
• Prior experience in home health setting preferred.
• Valid driver’s license and reliable transportation.

Responsibilities

• Visit patients in their homes and assess/implement therapy plans.
• Develop and update individualized care plans with physician orders.
• Monitor progress and adjust treatment to meet goals.
• Document care plans and communicate with the care team.
