Job summary

ABL Health Care in Oakley, CA is seeking a compassionate Occupational Therapist (OT) for our Home Health team. This Full-Time role requires active California OT licensure and driving within the service area to visit patients in their homes.

You will assess patient needs, develop and implement care plans, and collaborate with a clinical team to deliver high-quality in-home care. Travel is a regular part of the role, with strong organizational skills and patient focus valued.

Qualifications

• Current OT License to practice in California - Required.
• Experience in Home Health or Hospice care - 1 Year Required.
• Current CA Driver's License.
• Able to travel within a 15 - 25 mile radius on a regular basis.

Responsibilities

• Provide, coordinate and oversee skilled intermittent care to patients in the home.
• Evaluate patient status and develop/update care plans.
• Coordinate with clinicians and report changes to the team.
• Maintain accurate records and patient education for families.
