Job summary

Qualifications
- Missouri LPN license is required.
- At least one year of LPN experience (acute, long-term care, or home health preferred).
- Home health experience preferred.
Responsibilities
- Recognize and report changes in patient condition to the RN supervisor
- Complete timely, accurate clinical documentation
- Support and carry out the patient’s plan of care alongside the RN
- Collaborate with therapists and other clinicians to ensure continuity of care
