Job summary

The North Carolina Department of Health and Human Services (DHHS) is seeking an Early Intervention Service Coordinator to work with families and multidisciplinary teams to develop IFSPs and coordinate early intervention services across the CDSA.

The role involves home visits, coordinating therapies, and planning transitions for children aging out at three, with a focus on evidence-based natural learning environment practices and caregiver coaching.

Qualifications

• Experience working with children aged 0–5 and their families.
• Knowledge of child development milestones.
• Experience collaborating with families and professionals.
• Knowledge of evidence-based practices in natural environment settings and coaching.

Responsibilities

• Develop functional IFSPs with families and team members.
• Coordinate services with multi-disciplinary teams.
• Conduct home visits and monitor service delivery.
• Coordinate therapies (speech, physical, etc.).
• Arrange and monitor other early intervention services.
• Plan for transition to services at age three.
