Job summary

JD Home Healthcare is seeking a Direct Support Professional to provide comprehensive personal care and guidance to clients in Minnesota. You will promote independence, model appropriate behaviors, and help meet each client’s goals safely, with attention to individualized plans.

Qualifications

• Must be at least 18 years old.
• High School Diploma, GED, or equivalent.
• Ability to pass a background check with the MN Department of Human Services.
• General computer skills and knowledge.
• Reliable transportation insured in Minnesota; proof of insurance and current driver's license.
• Clear driving record (No DUI/DWI convictions in past 5 years).
• Fluency in reading, speaking, and writing English; strong communication skills.
• Working cell phone with internet access.

Responsibilities

• Assist clients in behavioral and personal care services.
• Assist clients with activities of daily living according to care plans.
• Accompany clients on recreation and leisure activities.
• Implement clients' plans as written.
• Increase clients' independence and involvement in their community.
• Understand and implement Behavior Support Plans and Individual Service Plans.
• Track and chart data daily in compliance with company policies.
• Attend and engage in team meetings as needed.
