Qualifications

• High School or equivalent diploma or one year of work experience preferred
• One year of work experience preferred

Responsibilities

• Assist individuals with daily living activities (personal care, meals, transportation) and support plans.
• Document progress and maintain accurate records.
• Facilitate community integration and safe, respectful living environments.
• Communicate with families and care teams to coordinate services.
• Attend program trainings with the QIDP and implement OSP/PCSP programs and learning objectives.
• Ensure medication oversight and health-related support as part of daily routines.
