Job summary

Sertoma Star Services seeks a Direct Support Professional in Chicago Heights to empower individuals with disabilities and mental illness. You’ll work in Community Day Services to build skills, promote independence, and enable meaningful daily participation.

Responsibilities include supportive training, documentation, and collaboration with families and staff. A valid driver’s license is required, and a Pace certification must be obtained within 3 months of hire.

Qualifications

• Must have a High School Diploma or GED.
• Must maintain a valid driver’s license and meet driving requirements when transportation is needed.
• Obtain Pace certification within 3 months of hire; maintain while in CDS program.
• Knowledge of working with individuals with disabilities and ability to communicate effectively.

Responsibilities

• Communicate with consumers using their preferred communication method to determine likes, interests and needs.
• Treat consumers with dignity and respect while promoting independence.
• Provide training and support to help consumers achieve Personal Plan/Implementation Strategy outcomes.
• Assist consumers in daily living and community participation.
• Document supports provided in HSMS and related records in a timely manner.
