Job summary

CHI Friendship is seeking a Direct Support Professional to provide compassionate, person-centered support to individuals with developmental disabilities, helping them live fulfilling lives and achieve their goals.

You will directly assist with daily living activities, implement support plans, document progress, and facilitate community integration while maintaining a safe, respectful environment and communicating with families and care teams.

Qualifications

• High School or equivalent diploma or one year of work experience preferred.
• One year of work experience preferred.

Responsibilities

• Documents in an accurate and timely manner in all applicable records, charts and files of people supported.
• Attends program training with the QIDP and accurately implements all OSP/PCSP programs, learning objectives, and supports. Attends team meetings and completes assessments as required
• Ensures each person within the department has all of his/her basic assurances met
• Assists people to remain connected to their natural supports and to be active members of the community
• Learns about the health needs and medications of the person supported and accompanies them to medical appointments
