Job summary

Qualifications

• High School diploma or equivalent; one year of work experience preferred.
• CPR/First Aid certification maintained or required.

Responsibilities

• Assists individuals with daily living activities (personal care, meals, transportation).
• Documents progress and implements support plans accurately.
• Attends and participates in team meetings and required assessments.
• Supports integration into community activities and resources.
• Administers medications and monitors health needs per care plans.
