Job summary

Monarch Healthcare is hiring an Assertive Community Treatment Team Peer Support Specialist to provide community-based, individualized services and promote self-determination. You will work within the ACTT to integrate participants' perspectives into treatment and rehabilitation, collaborating with staff to support recovery goals.

The role emphasizes symptom-management coaching, housing and transportation assistance, and community integration, leveraging lived experience and professional CPSS

Qualifications

• High school diploma is required.
• CPSS certification and valid US driver’s license required.
• Experience with adults with a mental health diagnosis and/or substance use disorder; lived experience related to mental illness or substance use is relevant.

Responsibilities

• Carry out duties as a member of the Assertive Community Treatment Team under the Team Leader.
• Participate in comprehensive psychiatric assessments and health education.
• Document participant progress to maintain permanent records.
• Attend meetings and training; assist with treatment planning.
• Provide individual, group, and family services to teach coping and self-management skills.
• Help participants access housing, transportation, and financial supports.
• Organize and lead social and recreational activities; support daily living activities.
