The role offers a competitive hourly pay range of $20.35 - $27.27.

Qualifications

• Bachelor’s degree from a four‑year college or university in a related field; or at least one year of related experience.
• Valid Connecticut driver’s license in good standing.
• Certification in physical/psychological management training, CPR and First Aid.

Responsibilities

• Coordinate intake process for new referrals.
• Implement the Individual Service or Recovery Plan (ISP/IRP).
• Develop and maintain relationships with community organizations.
• Prepare for and participate in team meetings.
• Empower individuals to maximize their input regarding services.
• Assist individuals with transportation as required.
