Job summary

Pacific Clinics is seeking staff to locate and place adults in volunteer work experiences and help them transition to competitive employment in the community. The role covers assessment of work readiness, barrier identification, and collaboration with therapists, case managers, and employers to build effective employment plans.

Overview

Pacific Clinics will be opening a high‑intensity, community‑based Adult Full Service Partnership (FSP) programs for adults age 18+ with serious mental illness (SMI) and complex needs. The program delivers “whatever‑it‑takes,” recovery‑oriented services primarily in the community to stabilize individuals, reduce hospitalizations and incarceration, prevent homelessness, and support long‑term recovery and community integration.

The ACT FSP (Assertive Community Treatment – Full-Service Partnership) program is an intensive, community‑based mental health service for 96 individuals with serious mental illness who have high needs. It provides wraparound, multidisciplinary support, including psychiatry, therapy, case management, crisis intervention, housing assistance, and rehabilitation, delivered with a community‑driven approach. The goal is to reduce hospitalization and incarceration, promote recovery, support housing stability, and improve quality of life through continuous, personalized care. This program will also serve those who are involved in the justice system as well.

Compensation We Offer
  • The initial compensation for this position ranges from $23.00 - $27.41/ per hour; depending on experience, location, and internal equity considerations.
  • 8% License Differential for LMFT, LCSW, LPCC, PsyD. and select Peer Certifications.

Job Summary

Responsible for locating and placing consumers in volunteer work experience positions in the community. Assists consumers with maximizing functional work behaviors and overcoming barriers to employment through training in appropriate work‑related behaviors. Facilitates consumers' transition from the Work Adjustment Program to the Employment Services Program and into competitive employment in the community.

Responsibilities
  • Engages consumers and assesses consumers' work readiness.
  • Assists consumers with identifying barriers they may have to obtaining employment in the community.
  • Collaborates with the consumer, the consumer's therapist, case manager, and other team members to develop and implement an employment plan that addresses overcoming the consumer's barriers to employment.
  • Assists/coaches consumers in the development of job search skills, transportation arrangements, resume preparation, interviewing techniques and job retention.
  • Teaches Peer Support Specialist courses and Work Readiness/Pre‑employment workshops.
  • Maintains listing of job opportunities and establishes relationships with prospective employers.
  • When appropriate visits work site, observes actual work in progress and prepares analysis of work environment, job requirements and specific job tasks.
  • Meets with consumers on a weekly basis or more frequently as needed.
  • Maintains ongoing liaison with supervisors obtaining feedback regarding consumer performance; assists in resolving problems quickly and efficiently.
  • Accompanies consumers to placement interviews, work sites, etc. as needed.
  • Keeps program staff informed about consumer performance and support needs and follows up to assure all issues are addressed.
  • Generates monthly reports reflecting consumer activity, specific problems and action taken to address the problems during that month.
  • Documents in progress notes consumer activity, communication, participation, contacts and concerns.
  • Attends meetings as assigned, including program employment meetings, service team meetings and staff meetings.
  • Develops and maintains accurate, complete and timely documentation of services provided to consumers; ensures compliance with Pacific Clinics policies and procedures, contract compliance, state licensing and other requirements; enters information in consumer records, gathers data and may prepares reports on for management use and decision‑making.
  • Develops and maintains skills in the employment field through seminars, workshops or other forms of training/education.
  • Interacts with other internal programs, clinical/support staff and external sources such as school or community personnel as applicable. Reports to work on time and maintains reliable and regular attendance. Models Pacific Clinics' approach, mission and core values in all communication and correspondence.
  • Communicates effectively in a culturally competent and diverse consumer population and promotes favorable interaction with managers, co‑workers and others. Performs other duties as assigned.
Qualifications

  • High School Diploma or GED and two (2) or more years of prior experience providing employment services to individuals who have relevant experience with mental/behavioral health condition, or equivalent combination of education and experience (two years of relevant experience equals one year of education) to include:
  • Certificate in the Mental Health Worker Course.
  • Demonstrated skills working effectively with the homeless and dually diagnosed population.
  • Knowledge of mental health and employment community resources.
  • A working knowledge of the Psychosocial Rehabilitation model.
  • Basic knowledge of public policies relative to client population and associated programs.
  • Ability to demonstrate advocacy skills when appropriate.
  • Bi‑lingual skills in Spanish/English.
  • Ability to work a flexible schedule as needed to fulfill program needs which may include weekends, evenings, and holidays.
Physical Requirements

While performing the duties of this job the employee is frequently required to stand or sit. The employee is required to use hands to produce records and/or documentation in manual or electronic format. The employee must regularly lift and/or move up to 5 pounds and occasionally move or lift up to 10 pounds. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
