Job summary

TURN is seeking a dedicated professional to support clients with work readiness, assessment, and intervention aimed at job attainment. You will provide case consultation, crisis intervention, discharge planning, and both individual and group work to help clients access essential employment opportunities.

The role involves documentation, coordination with a multidisciplinary team, field-based field services, and occasional on-call duties.

Qualifications

• Bachelor's degree with 1 year of related experience, or an associate degree with 3 years, or 5 years in a related field.
• High School Diploma or GED is required.
• Bilingual abilities are preferred; knowledge of behavioral health employment needs is advantageous.

Responsibilities

• Provide case consultation, crisis intervention, discharge placement, individual and group work, and assist clients in meeting basic needs and employment needs.
• Complete accurate and timely documentation as required and meet minimum requirements for direct billable client services.
• Advocate for clients to ensure work success and help link to income and benefits.
• Educate clients about competitive work opportunities and assess their needs.
• Offer direct placement of clients in positions aligned to skills and needs.
• Provide team with expertise on work trends, employer profiles, and demographic data as needed.
• Provide long-term job coaching for clients to assist in work stability.
• Ongoing client support, providing information and education to family and other supports.
• Facilitate placement including pre-placement visits and transportation as needed.
• Shift coverage including part of the on-call team.
• Provide field-based services as required by program and client need.
• Maintain confidentiality of client's care.
• Participates in scheduled program meetings.
• Other relevant duties as assigned.

QUALIFICATIONS

• Bachelor's degree and 1 year experience or associate degree and 3 years' experience or 5 years' experience in related field required. Need to verify this is required!
• High School Diploma or GED required.
• Knowledge of employment needs of individuals with behavioral health needs preferred.
• Cultural competency understanding required.
• Bilingual preferred.
• Knowledge of Microsoft Office and EHR software preferred.
