Job summary

Eden Health in Longview, WA is seeking a compassionate Registered Nurse to provide skilled home health care and manage patient plans under the Clinical Manager's supervision.

You will administer hands-on nursing care, coordinate with multidisciplinary teams, document patient progress, and ensure compliance with physician orders while maintaining patient and family engagement in the care plan.

Qualifications

• Graduate of an accredited nursing program.
• Current licensed RN in the state of practice.
• Experience: 1 year acute-care within last 24 months or 6 months Home Health within last 12 months.
• Valid driver’s license and reliable transportation.
• Proof of automobile insurance (MVR checked) and CPR certification.
• Able to pass criminal background check and national sex offender clearance.

Responsibilities

• Provide admission, case management and follow-up skilled nursing visits for Home Health patients.
• Administer ongoing care and case management for each patient under Clinical Manager supervision.
• Provide hands-on care, management and evaluation of the care plan and teaching of the patient.
• Coordinate care with Physical/Occupational/Speech Therapists, Social Workers and Home Health Aides; document patient progress.
