Eden Health is seeking a Registered Nurse for home health in Oregon. You will provide hands-on patient care, develop and adjust care plans, and coordinate with clinicians to ensure high-quality home health services.

Qualifications

• Graduate of an accredited nursing program.
• Current licensed Registered Nurse (RN) in the state of practice.
• Experience as an RN with home health preferred.
• Credentials in specialty areas preferred (wound care, OASIS certification, IV).

Responsibilities

• Provide hands-on home health care, administer and evaluate care plans.
• Coordinate with physicians and Clinical Manager for ongoing patient care.
• Document and report patient progress and care plans.
• Oversee home health aides and supervise care delivery as directed.
