Job summary

STLC in Charlotte, NC is seeking a Personal Care Aide (PRN) to perform patient care and activities in home as directed. Reports to the Senior TLC ADHC Coordinator and requires a high school diploma or equivalent.

At least 1 year as a PCA with frail elderly population is expected, plus a valid driving license with personal vehicle insurance. Training will be provided to meet CMS-approved competencies before working independently.

Qualifications

• High school diploma or equivalent.
• At least 1 year working as a PCA and with frail elderly population.
• Must have a valid driving license with personal vehicle insurance.

Responsibilities

• Provides personal care (bathing, dressing, feeding, toileting, transfers, lifting, ambulation and incontinent care) according to plan of care.
• Serves meals according to specific diets and performs incidental household duties (meal prep, housekeeping, laundry, errands).
• Obtains vital signs and reports changes in medical or physical status to supervisor.
• Documents services and observations on appropriate forms.
• Participates in on-call rotation for home care on weekdays and weekends.
