Job summary

Qualifications

• Active HHA certification in good standing in Pennsylvania.
• CNA license current in the state.
• At least one year of patient care experience preferred but not required.

Responsibilities

• Provide generalized aide care to assigned patients.
• Report patients' status, home environment, safety factors and family support to the team.
• Observe changes in patient condition and coordinate care with the physician and management.
• Assist patients with activities of daily living to promote independence.
• Maintain accurate patient records and reports.
• Participate in on-call rotation as necessary.
• Perform other related duties as assigned by management.
