Job summary

Ochsner Health seeks a Substance Use Navigator to engage ED patients with substance use disorders, providing trauma-informed, non-judgmental support and real-time referrals to treatment options.

You will coordinate with ED clinicians, social workers and treatment providers, document interactions in the EHR, and help patients access community resources and follow-up care after discharge.

• Commitment to patient-centered, trauma-informed, and culturally responsive care.

Responsibilities

• Conduct bedside engagement of ED patients identified with substance use concerns using trauma-informed and person-centered approaches.
• Establish rapport and build trust with patients, including those in crisis or ambivalent about treatment.
• Performs SBIRT activities.
• Educates patients and families on SUD, harm reduction strategies, and treatment options.
• Facilitates real-time referrals and warm handoffs to treatment programs and MAT providers.
• Coordinates with ED clinicians, social workers, and behavioral health teams to support care plans.
• Documents all patient interactions and referrals in the EHR.
• Assists with follow-up outreach to support continuity of care post-discharge.
• Participates in interdisciplinary team meetings and program improvement efforts.
