Ochsner Health in Louisiana seeks a Substance Use Navigator to engage ED patients with substance use concerns, perform SBIRT, and coordinate referrals to treatment and recovery services. You will document all interactions, support follow-up care, and help connect patients to MAT and recovery services while navigating complex social needs within a fast-paced emergency department

Formación

• High School Diploma or GED required.
• Associate or Bachelor's degree in related field preferred.
• Experience in healthcare, behavioral health, or community outreach preferred.

Responsabilidades

• Engage ED patients with substance use concerns using trauma-informed approaches.
• Build rapport with patients in crisis or ambivalent about treatment.
• Perform SBIRT activities and referrals to treatment.
• Educate patients and families about SUD and harm reduction options.
• Coordinate real-time referrals with MAT, peer support, and community resources.
• Document interactions and referrals in the EHR.
• Support follow-up outreach to ensure continuity of care.
