The Emergency Room RN Case Manager, in collaboration with internal and external interdisciplinary teams, coordinates and directs patient care throughout the emergency department stay and transition process. The role promotes safe, timely, and cost-effective care through comprehensive assessment, discharge planning, resource management, utilization review, patient advocacy, and collaboration with patients, families, providers, and community partners. This position supports optimal patient outcomes while ensuring appropriate utilization of healthcare resources and exceptional customer service.
