Job summary

Qualifications

Responsibilities

• Performs ongoing patient assessment and data collection across physiologic, psychosocial, cultural, spiritual and cognitive domains.
• Formulates and implements goal-directed care plans.
• Delivers care consistently and safely, coordinating with the health care team.
• Establishes priorities based on patient needs and available resources.
• Demonstrates knowledge of legal issues, patient confidentiality and risk management.
