Job summary

Qualifications

Responsibilities

• Assess data in a systematic manner focusing on physiologic, psychosocial, cultural and cognitive status.
• Formulate a goal-directed plan of care and implement it.
• Coordinate and deliver care with the health care team, ensuring patient safety and quality outcomes.
• Prioritize patient needs based on acuity, staffing, and resources.
• Educate patients and families; support discharge planning and continuity of care.
• Document patient care and unit activities timely and accurately.
• Demonstrate knowledge of legal and ethical considerations, including patient confidentiality.
