Job summary

The role requires documentation of assessments and discharge plans, coordination with care teams, and adherence to safety and infection control policies. CPR within 30 days is required, with strong communication skills.

Qualifications

• Current Licensed Social Worker (LSW) in the State of Ohio.
• CPR certification within 30 days of employment.
• Demonstrates competence in age-specific patient care as documented by the attached checklist.
• Aggression Management System within six months of employment.
• Ability to read, write and explain complex information to hospital staff.
• Attention to detail and strong communication skills.
• Ability to collect data, interpret findings and carry out plans.

Responsibilities

• Completes initial screenings of patients presenting in the ED for Behavioral Health services.
• Reviews clinical information and discusses with ED Physician and on-call Psychiatrist.
• Documents assessment and discharge plans in the medical record per standards.
• Verifies insurance coverage when applicable.
• Facilitates referrals to other facilities when appropriate.
• Provides leadership and coordinates care with other caregivers.
