Job summary

Harrison Memorial Hospital in Cynthiana, Kentucky, seeks a Social Worker for outpatient and emergency services on the day shift, full-time. You will perform psychosocial assessments, counseling, crisis intervention, and care navigation for patients and families in the ED and outpatient settings.

You will collaborate with physicians, nursing, care management, and community resources to address barriers to care, coordinate discharge planning, and ensure compliance with JCAHO, CMS, HIPAA and state

Qualifications

• Bachelor's degree in Social Work (BSW) is required.
• Strong knowledge of psychosocial assessment techniques, crisis intervention, and counseling principles.
• Knowledge of federal, state, and local healthcare resources; ability to navigate social services.
• Excellent interpersonal and communication skills with patients, families, staff and community agencies.
• Ability to work collaboratively in a fast-paced, emotionally charged environment.
• Strong organizational skills and ability to prioritize multiple complex cases.
• Commitment to patient advocacy, confidentiality, and ethical practice.

Responsibilities

• Provide professional psychosocial assessment, counseling, crisis intervention, resource coordination, care navigation, and social support in the ED and outpatient services.
• Collaborate with physicians, nursing, care management, outpatient providers, and other teams to address barriers to care and connect patients with hospital and community resources.
• Support compliance with Joint Commission (TJC), CMS, HIPAA, and state requirements related to patient rights, psychosocial services, care coordination, discharge planning, and access to services.
