Job summary

Qualifications

• Possession of a valid NY State license to practice pharmacy.
• Two years of post licensure experience; or completion of a post graduate residency or fellowship program; or current Pharmacy Board Certification.
• One year of post licensure supervisory or specialty clinical experience for Level III.

Responsibilities

• Fills prescriptions and patient orders, resolves discrepancies in patient therapy, and follows-up on problems.
• Utilizes clinical data in evaluating physician orders to ensure appropriate therapy.
• Provides and documents drug information and patient counseling.
• Documents interventions, ADRs, and medication errors.
• Coordinates and analyzes statistics and/or clinical data.
• Participates in quality management and utilization review programs.
