Job summary

Penn State Health, at St. Joseph Medical Center in Reading, PA, is seeking an Emergency Medicine Pharmacist.

You will manage pharmacotherapy in a fast-paced ED, collaborating with physicians and nurses to optimize medication therapy for both adult and pediatric patients. Responsibilities include rapid dosing and dosing support during trauma, stroke alerts, and cardiac arrests, along with ensuring safe transitions of care and accurate reconciliation at admission and discharge.

Qualifications

• Pharmacist must be licensed or eligible to practice in Pennsylvania.
• A PGY-2 Emergency Medicine or Critical Care residency with BCCCP or BCEMP required within one year of hiring.
• A PGY-1 general practice residency + three years of relevant experience and BCCCP or BCEMP required within one year of hiring.
• Six years of relevant experience and BCCCP or BCEMP required within one year of hiring.

Responsibilities

• Provide clinical and operational pharmacotherapy support in the ED for adult and pediatric patients.
• Assist in rapid medication dosing, drug selection, administration, and monitoring during resuscitations and critical events.
• Support transitions of care with medication reconciliation on admission and discharge.
