Johns Hopkins Intrastaff at Johns Hopkins Bayview Medical Center in Baltimore is seeking an Emergency Department Social Worker II (LMSW) to support patients and families in a fast-paced acute care setting.

You will perform psychosocial assessments, crisis intervention, discharge planning, and coordinate with interdisciplinary teams to promote safe transitions and equitable access to services.

Qualifications

• MSW from an accredited college or university.
• Current Maryland LMSW license.
• Minimum of two years in emergency department or hospital social work.
• Experience conducting psychosocial assessments and discharge planning in acute care.

Responsibilities

• Conduct comprehensive psychosocial assessments for ED patients and families.
• Provide crisis intervention, emotional support, and counseling in a high-volume ED.
• Perform safety assessments and interventions for behavioral health crises.
• Develop patient-centered discharge plans to support safe transitions of care.
• Coordinate referrals to community resources and social services.
• Collaborate with physicians, nurses, case managers, and behavioral health teams.
• Participate in interdisciplinary rounds and treatment planning.
• Advocate for patients and families to improve access to care.

Johns Hopkins Intrastaff, the internal staffing agency for the Johns Hopkins Health System and its partner hospitals, is seeking a compassionate and skilled Emergency Department Social Worker II (LMSW) to support patients and families at Johns Hopkins Bayview Medical Center.

In this fast-paced acute care environment, you will provide psychosocial assessments, crisis intervention, discharge planning, behavioral health support, and community resource coordination for patients presenting with complex medical, emotional, social, and behavioral health needs. Pay Rate: $47.00 - $49.00 per hour (based on experience)

Work Authorization: Candidates must be legally authorized to work in the United States and meet all Johns Hopkins onboarding and credentialing requirements.

Responsibilities

• Conduct comprehensive psychosocial assessments for emergency department patients and families to identify social, emotional, behavioral, environmental, and healthcare-related needs.
• Provide crisis intervention, emotional support, de-escalation, and counseling services in a high-volume Emergency Department setting.
• Perform safety assessments and assist with interventions related to behavioral health crises, homelessness, substance use disorders, domestic violence, and trauma.
• Develop and implement patient-centered discharge plans that support safe transitions of care and reduce barriers to treatment.
• Coordinate referrals to community resources, housing programs, behavioral health services, substance use treatment programs, financial assistance resources, and social service agencies.
• Collaborate with physicians, nurses, case managers, psychiatric teams, and community partners to support optimal patient outcomes.
• Participate in interdisciplinary patient care rounds, treatment planning meetings, and case reviews.
• Advocate for patients and families while promoting access to healthcare services and social support systems.
• Maintain timely and accurate documentation within the electronic medical record (EMR) in accordance with hospital and regulatory standards.
• Contribute to quality improvement initiatives, healthcare delivery enhancements, and social work best practices.
• Provide support and mentorship to social work students and interns as assigned.

Qualifications

Required:

• Master's Degree in Social Work (MSW) from an accredited college or university.
• Current Maryland Licensed Master Social Worker (LMSW) license.
• Minimum of two (2) years of experience in emergency department social work, hospital social work, crisis intervention, medical social work, behavioral health, case management, or discharge planning.
• Experience conducting psychosocial assessments and developing discharge plans within an acute care or healthcare setting.

Preferred:

• Experience in an Emergency Department, Trauma Center, or Acute Care Hospital.
• Experience working with patients experiencing mental health crises, substance use disorders, homelessness, domestic violence, or complex psychosocial needs.
• Knowledge of local community resources and social service networks.
• Experience conducting behavioral health assessments and safety planning.
• Experience supporting vulnerable and high-risk patient populations.
• Previous experience within an academic medical center or large healthcare system.
• Experience mentoring social work students or interns.
