Responsibilities

• Plans, implements, evaluates, and delivers patient care based on patient assessment to optimize outcomes and maximize available resources.
• Monitors, records, and communicates patient condition as appropriate.
• Collaborates across disciplines to coordinate patient care, including transfer, discharge, referral, and spiritual/psychosocial support needs.
• Educates patient and family appropriate to age, culture, condition, and circumstances.
• Maintains clinical skills through orientation, ongoing education, and practice.
• Follows Tribal Health policies and procedures.
