Qualifications

• Licensed in the state(s) of practice or eligible for reciprocity.
• Immunization and non‑vaccine injectable certification may be required.
• Credentialing and privileging required per department.

Responsibilities

• Oversees pharmaceutical care and designs medication therapy regimens based on biomedical literature.
• Provides patient and caregiver counseling on therapy, adverse effects, compliance, and administration.
• Evaluates and monitors pharmacotherapeutic regimens and documents appropriately.
• Reviews medication orders for accuracy and safety; supports medical emergencies when needed.
• Supports medication safety and quality improvement activities and reports events.
• Precepts pharmacy students and residents; mentors teammates and assists with evaluations.
• Ensures regulatory compliance and proper storage/handling of medications.
