Job summary

Tap Healthcare Solutions is seeking a seasoned Manager of ED & Observation Case Management to lead Memorial Hermann Health System’s Adult Medical Center in Houston, TX. You will oversee ED and Observation case management operations, coordinating with physicians, nursing leadership, and social work teams to optimize throughput and discharge planning.

The ideal candidate brings 5+ years in case management and leadership, a CCM/ACM certification, and either RN or LMSW licensure.

Qualifications

• Current Texas RN license or LMSW required.
• LCSW preferred for social work candidates.
• Case Management Certification (CCM, ACM, or equivalent) required.
• Five (5) years of experience in Utilization Management, Case Management, Discharge Planning, or Care Coordination.
• Three (3) years of hospital-based nursing or social work experience.
• Three (3) years of demonstrated leadership or management experience.

Responsibilities

• Oversee daily operations for Adult ED and Observation Case Management.
• Improve patient throughput, length of stay, and discharge planning processes.
• Lead and mentor case managers and social work professionals.
• Partner with physicians, nursing leadership, and interdisciplinary teams to optimize patient outcomes.
• Drive quality improvement initiatives while ensuring compliance with regulatory and organizational standards.
