Job summary

Qualifications

• Education: PharmD or BS in Pharmacy as required for licensure.
• Ohio license (RPh) with eligibility for board certification within 1 year.
• Experience: PGY-1 in acute care; hospital dispensing familiarity preferred.
• Knowledge of drug interactions and formulary policies.

Responsibilities

• Review medication use to optimize therapy per guidelines.
• Educate patients on discharge medications and assist with access.
• Monitor pharmacotherapy and document in the medical record.
