Responsabilités

• Assesses and analyzes patients' health status and documents assessments.
• Initiates discharge planning with initial admission assessment.
• Plans and coordinates patient care with patient, family, and health care team.
• Transcribes and directs physician orders and educates patients as needed.
• Monitors patient outcomes and communicates plans of care to the team.
