Qualifications

Responsibilities

• Assesses and analyzes patient health status on the unit.
• Performs ongoing assessments and documents findings.
• Initiates discharge planning with admission assessment.
• Plans patient care in collaboration with patient, family, and healthcare team.
• Transcribes and executes physician orders; provides patient education.
• Coordinates care and communicates plan of care to the team.
• Documents patient response and follow-up; evaluates outcomes.
