Qualifications

Responsibilities

• Assesses and analyzes patients' health status on the assigned unit.
• Performs ongoing assessments and documents findings per policy.
• Initiates discharge planning during admission assessment.
• Plans and coordinates patient care with patients, families and the healthcare team.
• Documents plan of care, interventions, and patient responses.
• Transcribes and implements physician orders and provides patient education.
• Evaluates patient outcomes and adjusts care plans accordingly.
