Job summary

UVA Health in Charlottesville is seeking an Emergency Department RN Case Manager to combine acute care expertise with discharge planning, care coordination, and real-time assessment.

You will work with physicians, nurses, social workers, rehab providers, and community partners to ensure patients receive the right care at the right time, with a weekday schedule and occasional weekend rotations.

Qualifications

• Education: BSN required within 5 years of hire or transfer into Case Management.
• Experience: 2 years of relevant experience required.
• Licensure: Licensed to Practice as a Registered Nurse in Virginia.

Responsibilities

• Assess patients early in their Emergency Department journey and identify care coordination needs.
• Apply independent clinical judgment to complex cases including medical, surgical, trauma, and critical care populations.
• Coordinate safe dispositions with physicians, nurses, social workers, inpatient teams, and community resources.
• Remove barriers to discharge while advocating for patients and families during critical moments.
• Participate in interdisciplinary collaboration and real-time care planning.
• Utilize Epic and evidence-based case management practices to support continuity, quality, and safety.
