JOB SUMMARY

The RN Case Manager serves as a leader of the complex discharge planning process and a valued member of the healthcare team. They collaborate with physicians and other members of the healthcare team to assess, plan, implement and evaluate proactive/complex/safe discharge plans. Collaborates with MSW for guidance with psychosocially complex cases. The RN Case Manager is also responsible for facilitating coordination of care. The RN Case Manager along with the other members of the healthcare team accepts responsibility, authority, and accountability for management of care. This shall be in accordance with the current policies and procedures, mission, vision, and values of UVA Health Northern VA Culpeper. Serves as a resource for discharge planning and care coordination for medically complex cases. Maintains current knowledge of available community resources/post‑acute care options.

QUALIFICATIONS
Education

Associate's Degree in Nursing required. 4 Year/Bachelor's Degree, BSN, preferred.

Experience

2 or more years of healthcare experience preferred. Case Management or related experience preferred. Refer to the Life Support Training Policy for additional details.

Licensure

Current RN licensure in state of employment required.

Additional Skills/Requirements

Required: Weekend and holiday coverage.

Preferred: Knowledge/Competence of Microsoft Office products and keyboarding.

  • Medical Knowledge: Provides primary health care and performs selective medical services under the direction of practice physicians.
    • Obtains complete history and physical data on patients.
    • Reviews patient records.
    • Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures needed.
    • Performs a complete physical exam and records findings.
    • Establishes presumptive diagnosis.
    • Orders appropriate laboratory and diagnostic procedures.
    • Performs independent evaluations and treatment procedures in emergency situations.
    • Provides monitoring and continuity of care between physician visits.
The incumbent may be asked to perform additional duties as assigned.
PHYSICAL DEMANDS

Medium to Heavy Physical Demand: The job requires constant standing, frequently walking, and bending/stooping. Proficient communicative, auditory, and visual skills. Attention to detail and ability to write legibly. Ability to lift/push/pull 100 lbs. For any weight over 35 lbs., use Safe Handling Equipment.   • The base compensation range for this role is $31.22 - $50.50 hourly. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education. Please note that the pay range does not include any applicable incentive compensation programs such as shift differentials, clinical ladders, and other incentives that may be available for those in eligible positions.
