Job summary

You will coordinate discharge planning, referrals, and services, ensuring regulatory compliance and high-quality patient outcomes while guiding families and staff through complex care transitions.

Qualifications

• License/Registration/Certification in RN required.
• Education: Bachelor’s degree in Nursing.
• Experience: 2 years in clinical nursing.

Responsibilities

• Identify target patients for ED CM interventions and develop appropriate plans.
• Provide continuity of care and discharge planning aligned with regulatory standards.
• Coordinate referrals and utilize community resources for post‑ED care.
• Document status reviews and discharge planning activities per standards.
• Collaborate with managers, physicians, and care teams on standards of care.
• Maintain knowledge of regulations and UM screening for ED discharge decisions.
• Educate ED staff on admission status, care coordination, and community resources.
• Demonstrate age-appropriate patient care across the lifespan.
