Job summary

Adventist Health Glendale is seeking a dedicated RN Case Manager to optimize patient throughput in the Emergency Department and coordinate care across the continuum. You will work with patients, families, physicians, and care teams to determine appropriate levels of care and to ensure efficient, cost-effective care delivery.

Qualifications

• Bachelor's Degree in Nursing (BSN) preferred
• Master's Degree in nursing or health care related field preferred
• Experience in case management, utilization management, and/or transitional care preferred

Responsibilities

• Acts as a resource to ED staff and physicians regarding appropriateness of admission, levels of care and documentation requirements.
• Gathers information from relevant sources to facilitate discharge from ED to appropriate level of care.
• Assists in triaging calls from other facilities requesting patient transfers and determines transfer appropriateness.
• Organizes, integrates, and evaluates the plan of care and progress toward outcomes, communicating changes to patient/family and team.
• Performs other job-related duties as assigned.
