Job summary

Adventist Health Glendale seeks a Registered Nurse Case Manager for the Emergency Department to coordinate patient throughput, collaborate with the interdisciplinary team, and facilitate discharge planning across the continuum of care. The position requires an RN license in the state, with BSN/MSN preferred and prior case management experience.

Qualifications

• RN licensure required in the state of practice.
• BSN preferred (Bachelor of Science in Nursing).
• MSN preferred (Master of Science in Nursing) or health care related field.
• Experience in case management, utilization management, and/or transitional care preferred.

Responsibilities

• Acts as a resource to ED staff and physicians regarding admission appropriateness and levels of care and related documentation.
• Gathers information from relevant sources to facilitate discharge from ED to appropriate level of care in a safe, timely, and cost-efficient manner.
• Assists in triaging calls from other acute care facilities requesting patient transfers to EMC and determines transfer appropriateness.
• Assesses ED patients and identifies options other than acute hospital admission when appropriate; screens referrals to rehab, LTC, clinics, or home-based care as needed.
• Organizes, integrates, and evaluates the care plan; communicates changes to patient/family and care team.

Education
