Qualifications

• Licensed Physical Therapist in Illinois or eligibility.
• Genuine interest in working with older adults.
• Therapy should be functional, purposeful, and outcomes-driven.
• Builds relationships with residents, families, and interdisciplinary teams.
• Communicates exceptionally well and takes ownership of community outcomes.

Responsibilities

• Provide high-quality physical therapy to residents across Independent Living, Assisted Living, and Memory Care.
• Complete comprehensive evaluations and develop individualized plans of care.
• Treat residents with a focus on function, independence, and quality of life.
• Assist residents recovering after hospitalization, illness, injury, or functional change.
• Address balance, gait, strength, mobility, fall risk, endurance, and functional performance.
• Collaborate with caregivers and staff to apply therapeutic strategies in daily life.
• Educate residents and families on the rationale behind therapy.
• Document efficiently using the technology platform and AI-enabled tools.
• Contribute to a clinical culture built on learning, collaboration, and high standards.
