Optum seeks a Behavioral Health Clinician to be embedded in Summit Medical Group's primary care setting in Greenville or Morristown, TN, operating within a Collaborative Care Model (CoCM). Qualifications

• Active, unrestricted behavioral health license in Tennessee (LCSW, LMSW, LPC-MHSP, LMFT)
• 1+ year of behavioral health treatment experience
• Experience assessing and treating common mental health conditions
• Comfort delivering brief, evidence-based interventions
• Interest in multidisciplinary, team-based care
• Ability to work with diverse patient populations

Responsabilités

• Provide brief, evidence-based behavioral health interventions in person and via telehealth
• Screen, assess, and diagnose common mental health and substance use disorders
• Collaborate closely with primary care providers, psychiatric consultants, and care team members
• Track patient progress using measurement-based care and adjust treatment as needed
• Support psychotropic medication management by monitoring adherence and side effects
• Participate in regular psychiatric case consultations and communicate recommendations
• Develop relapse prevention and self-management plans for discharge
• Maintain accurate documentation following collaborative care workflows
