Job summary

Optum Behavioral Care in Greeneville, TN seeks a Behavioral Health Clinician embedded within Summit Medical Group’s primary care practice. You’ll deliver brief, evidence-based behavioral health care to adults, using CoCM, with collaboration among therapists, PCPs, and psychiatric consultants.

You will work in a hybrid setting (4 days onsite, 1 day remote), with a structured schedule Monday–Friday, no weekends. Telehealth and in-person care are supported to meet patient needs.

Qualifications

• Active, unrestricted behavioral health license in Tennessee (LCSW, LMSW, LPC-MHSP, LMFT).
• 1+ years of experience providing behavioral health treatment.
• Experience assessing and treating common mental health conditions.
• Proven comfort delivering brief, evidence-based interventions.
• Multidisciplinary, team-based care model experience.
• Diverse patient populations across mental health and medical conditions.

Responsibilities

• Provide brief, evidence-based behavioral health interventions in person and via telehealth.
• Screen, assess, and diagnose common mental health and substance use disorders.
• Collaborate with primary care providers, psychiatric consultants and care team.
• Track patient progress using measurement-based care and adjust treatment.
• Support psychotropic medication management and monitor adherence.
• Participate in regular psychiatric case consultations and communicate recommendations.
• Develop relapse prevention and self-management plans for discharge.
• Maintain accurate documentation and follow collaborative care workflows.
