Elliot Rehabilitation Services outpatient department includes 8 clinics across the greater Manchester area, offering physical, occupational, and speech therapies across various specialties including orthopedics, neurological, pelvic floor, vestibular, geriatrics, pediatrics, and oncology. About The Job

The physical therapist evaluates, plans, and implements treatment programs for patients, based on principles of the discipline and setting (acute or outpatient).

Responsibilities

• Conduct medical record reviews and prepare for patient evaluations, ensuring therapy orders are active and appropriate.
• Assess patient needs and develop a written plan of care with achievable goals, collaborating with patients and healthcare team members.
• Re-evaluate and update treatment plans, communicate with referrers, and support discharge planning including patient and family education on home exercises and equipment use.
• Supervise therapy assistants and other personnel involved in patient care.
• Participate in clinical development programs, including mentorship, educational presentations, and community outreach.

Qualifications

• Graduate of an accredited Physical Therapy Program with an Entry Level Degree.
• Licensed as a Physical Therapist in New Hampshire.
• CPR certification (or able to obtain within 90 days).
• Previous clinical experience preferred but not required; recent graduates are eligible.
