Qualifikationen

• Minimum 2 years experience in document management or service coordination.
• Experience in health or social services preferred.
• Strong data entry and accuracy in maintaining records.
• Bilingual English/Spanish highly preferred.
• Familiarity with MAP, Medicaid, CHIP, ADAP programs.

Aufgaben

• Interview and screen applicants in person or by phone to collect demographic, household, and financial information to determine eligibility for healthcare programs.
• Assist clients with completing and submitting applications and required documentation, including renewals and follow-ups.
• Collect, review, and upload client eligibility documents into designated portals such as VeritySource, Take Charge Texas, Provide Enterprise.
• Monitor timelines and renewal dates to prevent lapses in eligibility and coordinate with teams to resolve missing documents.
• Maintain organized and confidential client records in approved databases and software.
• Provide general assistance to clients visiting eligibility offices and refer them to appropriate services.
• Troubleshoot and follow up with external agencies to support application completion.
• Attend required trainings on eligibility programs and policy changes.
• Support intake registrations in funder databases as patients enter HIV care.
• Participate in audit and quality assurance of client files and determinations.
• Represent the organization at community events as needed.
