Job summary

Amergis is seeking an Occupational Therapist (OT) to support an elementary school caseload in Maricopa, AZ. The OT will evaluate students, create treatment plans, deliver therapy, and document progress per each plan of care.

Minimum requirements include AZ OT licensure, AZ DOE OT Certification, FPCC, CPR/First Aid, and preferred school-based or pediatric experience. This is a full-time, 40-week contract with on-site duties.

Qualifications

• Active OT licensure in Arizona or ability to obtain.
• AZ Department of Education OT Certification required.
• Arizona IVP Fingerprint Clearance Card (FPCC) required.
• CPR/First Aid Certification required.
• School-based or pediatric experience preferred.

Responsibilities

• Perform student evaluations and develop treatment plans.
• Provide OT therapy services to elementary students.
• Maintain documentation in accordance with each student's plan of care.
